What this marker is

This ratio compares testosterone, which drives tissue building, against cortisol, which mobilises energy by breaking tissue down. Both are necessary. What the ratio describes is which direction the balance currently sits in.

It is used in sports science as an indicator of recovery capacity and training load tolerance. A falling ratio across a training block is one of the signals associated with accumulated fatigue rather than productive adaptation.

Because it is derived from two hormones that both follow daily rhythms, sampling time affects the ratio as much as it affects either input. Comparisons are only meaningful between samples taken at similar times.

If your result is low

Below 3.5 means cortisol is high relative to testosterone, which is the catabolic direction. This is the result associated with sustained stress, insufficient recovery, or training volume that has outrun the capacity to adapt to it.

Reported effects include training performance that stalls or declines despite consistent effort, persistent fatigue, disturbed sleep and reduced motivation.

Undereating deserves specific mention, because it moves both sides in the wrong direction at once. Insufficient calories suppress testosterone and raise cortisol, so a low ratio in someone training hard on restricted intake often reflects the intake rather than the training.

If your result is in range

Between 3.5 and 15 your ratio sits inside the reference range for this panel.

The trend over repeated tests carries more information than a single value. A ratio drifting down across a training block suggests recovery is not keeping pace, even while each individual result remains in range.

If your result is high

Above 15 means testosterone is high relative to cortisol. This is not automatically favourable, and the question is which side is producing it.

Unusually low cortisol raises the ratio without testosterone being high, and low morning cortisol has its own significance worth examining rather than reading as a good ratio.

Exogenous testosterone raises it from the other direction. A high ratio is worth interpreting through its two inputs rather than on its own, and worth discussing with your practitioner.

Why the Western lifestyle moves it

Chronic stress raises cortisol while suppressing testosterone, so the Western pattern pushes both inputs the same way and the ratio falls faster than either number alone would suggest.

Short sleep compounds it directly. Most testosterone release happens during sleep, and sleep restriction raises next-day cortisol, so a sleep deficit moves the numerator down and the denominator up at the same time.

What to eat

Eat enough. Undereating is the single most common driver of a low ratio in people who train, and no supplement compensates for a sustained calorie deficit.

Adequate protein and dietary fat support the anabolic side, since fat is the substrate for steroid hormone synthesis.

Zinc and magnesium rich foods support testosterone production, and stable blood glucose across the day reduces cortisol excursions.

Supplement forms and doses

Ashwagandha as KSM-66 at around 600 mg daily is the option that addresses both sides, lowering cortisol while supporting testosterone.

Magnesium glycinate at 300 to 400 mg in the evening supports sleep quality and recovery. Zinc bisglycinate at 15 to 30 mg supports the testosterone side.

Phosphatidylserine at around 300 mg is used to blunt exercise-induced cortisol. None of these substitute for reducing training load or extending sleep, which are the interventions that move this ratio most.
