Total Testosterone
The full circulating amount of testosterone, both bound and free.

What this marker is
Total testosterone counts all the testosterone in your blood, which is mostly not available to tissue. The large majority is bound tightly to sex hormone binding globulin, a smaller fraction is bound loosely to albumin, and only a few percent circulates free.
That matters for interpretation. A change in binding globulin shifts total testosterone without changing what tissue actually receives, which is why total and free results sometimes tell different stories.
Testosterone is built from cholesterol, so dietary fat is the raw material for the pathway. It supports muscle mass, bone density, red blood cell production, libido and mood in all sexes, at different concentrations.
If your result is low
Below 260 ng/dL is under the reference range for this panel. Reported effects include low energy, reduced training response, low libido, low mood and loss of muscle mass despite unchanged training.
Sampling time is the first thing to check. Testosterone follows a daily rhythm with a morning peak, so an afternoon sample reads meaningfully lower without indicating anything about production.
Sleep is the modifiable factor with the largest effect. Most testosterone release happens during sleep, and restricting sleep to five hours reduces daytime levels measurably in healthy young men. Chronic stress, undereating and excess body fat all suppress production through separate routes.
If your result is in range
Between 260 and 920 ng/dL your total testosterone sits inside the reference range for this panel.
The range is wide, and the same number can sit comfortably for one person and low for another. Symptoms alongside a number in the lower part of the range are worth raising rather than dismissing because the value is technically in range.
If your result is high
Above 920 ng/dL is above the reference range. In someone not taking anything, this is uncommon and worth investigating rather than celebrating.
Exogenous testosterone is the common explanation. Supplements marketed for testosterone support occasionally contain undeclared hormones, which is a route people do not always know they are on.
In women a high result carries different significance and can accompany conditions affecting ovulation. Either way, a high result without an obvious cause is a conversation for your practitioner.
Why the Western lifestyle moves it
Population testosterone has fallen across decades in ways that age alone does not explain. The candidates are the same Western factors that appear across this panel: short sleep, chronic stress, rising body fat, sedentary work and inflammatory dietary patterns.
Endocrine-disrupting compounds from plastics and personal care products are an additional exposure route that did not exist at the same scale a few generations ago.
What to eat
Eat enough fat. Steroid hormones are built from cholesterol, and very low fat diets reduce testosterone. Olive oil, avocados, eggs, nuts and grass-fed meats all work.
Zinc-rich foods matter directly, since zinc is required in the synthesis pathway. Oysters, beef and pumpkin seeds are the practical sources.
Cruciferous vegetables support oestrogen metabolism, which is part of the same hormonal picture. Eating enough total calories matters too, since sustained underfeeding suppresses production.
Supplement forms and doses
Zinc at 15 to 30 mg as bisglycinate and magnesium at 300 to 400 mg as glycinate are the foundational minerals, and both are worth correcting before anything else.
Vitamin D3 at 3,000 to 5,000 IU daily where the vitamin D result is low. The hormonal effect is secondary to simply not being deficient.
Ashwagandha as KSM-66 at around 600 mg daily has the most human evidence behind it, acting largely by lowering cortisol. Tongkat ali and boron are used for free testosterone specifically, with thinner evidence. Sleep does more than any of these.
