Vitamin E (Alpha-tocopherol)
The main fat-soluble antioxidant guarding your cell membranes from oxidation.

What this marker is
Vitamin E is the primary fat-soluble antioxidant in the body. Its job is specific: it sits inside cell membranes and stops the chain reaction that occurs when a polyunsaturated fat is oxidised. One oxidised fat would otherwise propagate damage to its neighbours, and vitamin E is what interrupts that.
This panel measures alpha-tocopherol, the form the body retains preferentially. Vitamin E is actually a family of eight compounds, four tocopherols and four tocotrienols, and the others have their own functions that a single alpha-tocopherol measurement does not capture.
Because it protects polyunsaturated fats, requirement rises with polyunsaturated fat intake. Someone eating a lot of seed oils needs more vitamin E than someone who is not.
If your result is low
Below 11 mg/L indicates low alpha-tocopherol. Membranes carrying polyunsaturated fats are less protected from oxidation, which matters most in tissues rich in those fats, including nerve tissue and red blood cells.
Fat absorption is the usual constraint rather than intake. Vitamin E needs dietary fat and adequate bile to be absorbed, so conditions affecting fat digestion produce low results despite reasonable diets.
High intake of oxidised or fried oils raises demand at the same time as it supplies little usable vitamin E, so the deficit can come from the demand side.
If your result is in range
Between 11 and 42 mg/L your alpha-tocopherol sits inside the reference range for this panel.
Serum vitamin E tracks blood lipids, so a result should be read with that in mind. Someone with very low circulating lipids can show a low vitamin E result without a genuine shortfall in tissue.
If your result is high
Above 42 mg/L is nearly always supplementation, often at the high doses that were popular when vitamin E was taken in isolation.
High-dose alpha-tocopherol alone displaces gamma-tocopherol, which has antioxidant functions alpha-tocopherol does not cover. Taking a large amount of one member of the family can therefore lower your position on another.
Vitamin E at high intake also affects clotting, which matters alongside anticoagulant medication. Review a high result and your total dose with your practitioner.
Why the Western lifestyle moves it
The Western diet raises vitamin E demand and supplies it poorly at the same time. Seed oils deliver large amounts of the polyunsaturated fats that vitamin E has to protect, while frying and processing degrade the vitamin E those oils originally contained.
Oxidative load from processed food, poor sleep and low antioxidant intake draws down stores faster than a diet built on whole foods would.
What to eat
Sunflower seeds, almonds and hazelnuts are the densest ordinary sources. Avocados contribute meaningfully.
Wheat germ oil is the richest food source by a wide margin. Extra-virgin olive oil supplies vitamin E along with fats that need less protecting than polyunsaturates do.
Dark leafy greens including spinach and Swiss chard contribute. Reducing fried food lowers demand from the other side.
Supplement forms and doses
A mixed tocopherol and tocotrienol complex at 200 to 400 IU daily rather than synthetic alpha-tocopherol on its own. Full-spectrum products supply gamma and delta tocopherol, which alpha-tocopherol alone displaces.
Take it with a meal containing fat. Absorption without dietary fat is poor.
Vitamin E and selenium work together in antioxidant defence, so the two results are worth reading side by side.
